Definition of Provigil

Provigil is a brand name for modafinil, a medicine that promotes wakefulness without behaving like a conventional stimulant. It is prescribed for excessive daytime sleepiness in narcolepsy, in obstructive sleep apnea alongside treatment of the airway itself, and in shift work sleep disorder. It reduces the sleepiness rather than correcting whatever is producing it.

How does it differ from a stimulant?

Amphetamine-type stimulants force dopamine and noradrenaline out of nerve endings throughout the brain, producing alertness together with a faster heart, lost appetite and a strong pull toward misuse. Modafinil raises dopamine as well, by occupying the transporter that normally clears it away, but it does so weakly and slowly, and it also acts on the histamine and orexin systems that hold wakefulness in place. The result is alertness with less physical stimulation and a lower, though not absent, potential for misuse; it remains a controlled substance. Its full mechanism has never been settled, which is why descriptions of it stay cautious.

In narcolepsy the illness stems from the loss of the nerve cells producing orexin, the signal that keeps wakefulness stable, which is why sleep intrudes into the day and why rapid eye movement sleep arrives abnormally early and leaks into waking as dream imagery and sudden weakness. Modafinil does not replace that missing signal. In apnea of the obstructive kind the airway closes repeatedly during the night, and the drug is added only for sleepiness that remains after the airway has been dealt with.

What limits its use?

It speeds up several liver enzymes that break other medicines down, which lowers the blood level of hormonal contraception enough that the contraception can fail, and it interacts with a range of other drugs by the same route. Serious skin reactions have been reported rarely, and headache is the commonest ordinary side effect, followed by nausea, anxiety and difficulty sleeping at night. Sleepiness that comes simply from not sleeping enough is not what the drug is licensed for. The label frames it as an addition to treatment of the underlying sleep disorder rather than a substitute for it.
