Definition of Pravachol
Pravachol is a brand name for pravastatin sodium, a cholesterol-lowering drug of the statin class. It blocks HMG-CoA reductase, the enzyme that sets the pace of cholesterol manufacture in the liver, which prompts liver cells to pull more LDL cholesterol out of the blood. It is taken as a tablet once a day, to lower cholesterol and to reduce cardiovascular events.
How does a statin lower cholesterol?
Indirectly, which surprises people who expect it to dissolve cholesterol in the blood. Most cholesterol in circulation is manufactured by the liver rather than eaten, and one enzyme controls the slowest step of that assembly line. Blocking it leaves the liver cell short. The cell responds by pushing more receptors for LDL onto its surface and hauling LDL particles in from the blood to make up the shortfall, and it is that scavenging, not the block itself, that drops the blood level. The same class of drug also calms inflammation in artery walls and steadies fatty deposits, effects that appear to matter separately from the numbers.

Because it dissolves in water rather than fat, pravastatin does not drift into cells generally; it is carried into liver cells by a specific transporter. It also bypasses the CYP3A4 pathway that many other medicines and grapefruit juice tie up, so the interaction list is shorter than for the fat-loving statins. Shorter is not empty. Ciclosporin blocks the transporter and pushes levels up, and gemfibrozil raises the chance of muscle injury alongside any statin.
What problems are associated with the class?
Muscle aching is the complaint that most often stops treatment, though careful trials find it far less specific to the drug than reputation suggests. Genuine muscle injury with released muscle protein is uncommon, and severe breakdown of muscle is rare. Liver enzyme rises are usually mild and settle. Across the class there is a small increase in new diagnoses of diabetes, concentrated in people already close to the threshold. The salt form dispensed is pravastatin sodium, and generic tablets contain the identical compound.
