Pancreatic means relating to the pancreas, the soft gland lying behind the stomach across the back of the upper abdomen. The two sit side by side but answer to different signals. Digestive enzymes are released when food arrives in the intestine, and the protein digesting ones travel in an inactive form so they do not eat the gland that made them. Islet hormones respond to blood sugar: insulin brings it down after a meal, glucagon raises it during a fast. Because the islets are scattered through the exocrine tissue, damage that is severe enough eventually costs both functions.
What goes wrong with it?
Acute pancreatitis is sudden inflammation, most often from gallstones blocking the shared outlet or from heavy alcohol use, in which the pancreatic enzyme supply activates early and digests the gland itself. Repeated attacks scar it, and a scarred gland produces too little enzyme, leaving fat undigested and passing into the stool. Cysts are common and mostly harmless, though a minority carry a risk of becoming malignant. Pancreatic cancer usually arises in the duct lining and tends to be found late because the gland sits deep and gives few early signals. Tumors of the islet cells are far rarer and often announce themselves through the hormone they oversecrete.
How is the pancreas examined?
Blood levels of two enzymes rise sharply in acute inflammation, and one of them is the more specific of the pair. Cross sectional imaging shows the gland, the duct and any mass. Ultrasound performed from inside the stomach with an endoscope gives the closest view and allows a needle biopsy in the same sitting. A stool test measures how much enzyme is reaching the intestine when digestive failure is suspected.
