Which muscles are used, and why those?
The deltoid at the shoulder is easy to reach and suits a small volume, which is why most adult vaccines go there. The outer thigh holds much more, is bulky even in infants, and is the usual site in young children and for larger volumes. The hip can also be used, and the preferred landmark there sits toward the side rather than in the upper outer buttock, because the sciatic nerve and large vessels run behind the older site. Needle length matters as much as the landmark: too short and the dose lands in fat, too long and it can reach bone.

Vaccines are the commonest example, since muscle contains the immune cells that pick up antigen and start a response. Any medication broken down by the stomach or the liver before it can work may take this route instead. So can long acting depot preparations, formulated to leave a reservoir in the muscle and release drug steadily for weeks, an option no intravenous dose can offer. Emergency drugs are sometimes given intramuscular because a vein is hard to find quickly and the thigh is not.
