Definition of gallbladder
The gallbladder is a small, pear-shaped organ tucked beneath the liver on the right side of the upper abdomen. It stores and concentrates bile, the fluid the liver produces continuously to help digest fat and to carry certain waste products out of the body. It squeezes that store into the intestine when a meal arrives.
How does the timing work?
The liver makes bile constantly, but fat only needs it at intervals, and the gallbladder resolves that mismatch. Between meals a muscular ring at the lower end of the bile duct stays closed, so bile backs up along the cystic duct into the gallbladder. There the wall absorbs water and salts, concentrating the bile several times over into a smaller, more potent volume. When fat and protein reach the small intestine, the intestinal lining releases a hormone called cholecystokinin into the blood. That hormone makes the gallbladder contract and the ring at the end of the duct relax, so concentrated bile travels down the common bile duct exactly when digestion calls for it.
What does bile actually do?
Bile salts work like a detergent. Fat does not dissolve in the watery contents of the intestine, so it would otherwise sit in large globules that digestive enzymes could only attack at the surface. Bile salts break those globules into a fine suspension of tiny droplets, hugely increasing the surface area available to enzymes and allowing the products to be absorbed. Bile also serves as a disposal route, carrying bilirubin, the pigment left from broken-down red blood cells, along with cholesterol and some drug byproducts, out of the body through the intestine.
