Definition of fibromyalgia

Fibromyalgia is a long-term condition of widespread pain and tenderness through the body, accompanied by unrefreshing sleep, persistent fatigue and difficulty with memory and concentration. The muscles and joints themselves are not damaged; the nervous system amplifies ordinary signals so that pressure and movement register as pain. Symptoms fluctuate, and onset often follows an illness, an injury or prolonged stress.

If nothing is damaged, where does the pain come from?

From the way pain signals are handled rather than from the tissue reporting them. Pathways in the spinal cord and brain that normally turn incoming signals down instead turn them up, a state described as central sensitization. Pressure that would register as touch registers as pain, and pain that should fade continues. Studies have found altered levels of the chemical messengers carrying and dampening these signals, and disrupted deep sleep appears to feed the process rather than simply result from it. What follows is genuine chronic pain with a mechanism behind it, not pain without a cause.

How is it recognized?

By pattern, because no scan or blood test confirms it. The picture is pain on both sides of the body, above and below the waist, lasting months, together with fatigue, unrefreshing sleep and difficulty concentrating that people often call brain fog. Assessment counts how many body regions are painful and rates the severity of the accompanying symptoms. Blood tests are performed to look for other explanations rather than to prove this one, and they return normal. An older approach counted tender points at fixed sites; it has largely been set aside, since widespread pain rather than tenderness at particular spots is what defines the condition.
