Definition of estrogen receptor
An estrogen receptor is a protein that binds estrogen and, once bound, acts on DNA to switch genes on or off. Most of it sits inside the cell, in the nucleus and the cytoplasm, with a smaller amount associated with the cell membrane. Two main forms exist, alpha and beta, distributed differently across the tissues of the body.
How does the receptor work?
Estrogen is a steroid and dissolves in fat, so it passes straight through the cell membrane rather than signaling from outside. Inside, it binds the receptor, which changes shape, pairs with a second receptor and settles onto specific stretches of DNA called estrogen response elements. There it recruits helper proteins that either start or silence the reading of nearby genes. The effect is slow compared with a hormone acting at the cell surface, taking hours rather than seconds, but it is far-reaching, because one hormone binding alters the production of many proteins at once. The smaller population of receptors near the membrane produces faster effects through separate signaling routes.

The alpha form predominates in reproductive tissue and the liver, the beta form in areas such as the colon, the immune system and parts of the brain, and the two can pull in opposite directions within one tissue.
Why is it tested in breast cancer?
Because most breast cancers keep the receptor and use it, and those tumors respond to treatment that removes or blocks estrogen. The estrogen receptor test on a biopsy sample therefore determines whether endocrine treatment belongs in the plan at all. It is reported alongside the progesterone receptor and HER2, and the combination rather than any single marker shapes what follows. The same receptor matters outside cancer as well, since drugs designed to act on it selectively are used for bone protection and for symptoms after menopause, and understanding it explains why one hormone affects breast cancer, bone and blood vessels through a single mechanism.
