Definition of disinfectant
A disinfectant is a chemical or physical agent applied to non-living surfaces and equipment to destroy most disease-causing microorganisms on them. Bleach, alcohol solutions, hydrogen peroxide and quaternary ammonium compounds are common examples. Disinfection kills most germs but does not reliably destroy bacterial spores, which is what separates it from sterilization.
How does it differ from an antiseptic or sterilization?
The words are often used loosely, but they describe different jobs with different strengths.

Why does the surface have to be clean first?
Blood, mucus and grease shield microorganisms from the chemical and can inactivate it outright. Bleach in particular is neutralized by organic material. This is why cleaning comes before disinfecting rather than instead of it. Contact time matters just as much: a surface wiped and immediately dried has not been disinfected, because the chemical needs to stay wet on the surface for a set period to work. Dilution matters too, since a solution made weaker than intended may reduce numbers of bacteria without killing them, and repeated exposure to weak disinfectant can select for organisms that tolerate it.
What resists disinfection?
Not every organism is equally vulnerable. Bacterial spores are the toughest, which is why an agent that kills ordinary bacteria may leave Clostridioides difficile behind and allow infection to persist in a ward. Certain viruses without an outer fatty envelope, such as norovirus, resist alcohol, since alcohol works largely by dissolving that envelope. Some bacteria form biofilm, a slime layer that the chemical cannot penetrate. Heat, ultraviolet light and other physical methods are used where a chemical cannot reach or would damage the item, and the choice of agent depends on which organisms are the concern.
