A coffee enema is the introduction of brewed, cooled coffee into the rectum and lower bowel through a tube. It is promoted within certain alternative treatment regimens on the claim that it detoxifies the liver. That claim has no support in physiology, and the practice carries documented risks including burns, bowel injury, infection and disturbed body salts.
Where did the practice come from?
Enemas of plain water have a long history in medicine as a way of clearing the lower bowel. Coffee entered the picture through alternative cancer regimens in the twentieth century, most prominently the Gerson protocol, which combined juice diets with frequent coffee enema use. The rationale offered was that caffeine absorbed through the bowel wall opens the bile ducts and prompts the liver to release toxins. Enzyme names are often cited alongside, giving the explanation a technical surface it does not have underneath.

What harms have been reported?
Case reports in the medical literature describe scalding of the rectal lining from coffee that was too hot, inflammation of the rectum and colon, perforation of the bowel wall, and bloodstream infection from equipment or from injury to the lining. Repeated use flushes out sodium, potassium and magnesium, and severe disturbances of body salts have been reported, including cases ending in death. The bowel also becomes reliant on the stimulus when enemas are used frequently over a long period.
How does it differ from a medical enema?
A medical enema uses water, salt solution or a specific preparation to empty the lower bowel before an examination or an operation, or to relieve severe constipation. Its purpose is local and mechanical. A barium enema introduces contrast so the bowel outline can be photographed. None of these are given with the intention of treating disease in another organ.
