What does the suffix signify?
Tumor names encode the tissue a growth came from. A carcinoma arises from epithelium, the sheet of cells that covers surfaces and lines tubes. A sarcoma arises from connective tissue, the supporting material built from muscle, fat, fibrous tissue, cartilage, bone and blood vessel walls. Applied to the bile duct, the two names therefore point at different layers of the same tube: one at the lining, the other at the wall beneath and around it. Because the lining is where nearly all bile duct cancers begin, sarcomas of this site are exceptionally uncommon, and much of what is understood about them is borrowed from the study of soft tissue sarcoma elsewhere in the body.

Why can imaging not settle it?
A scan shows a mass, its size, its position along the duct, and whether bile flow is obstructed. It cannot show which cell type the mass is built from, and a sarcoma and a carcinoma at the same site can produce an identical picture: a narrowed duct, a dilated system above it, and a soft tissue mass. Diagnosis therefore depends on tissue. A biopsy is examined under a microscope and stained for proteins characteristic of particular cell families, which is what separates a growth of the lining from a growth of the wall. The distinction is far from academic, since sarcomas and carcinomas differ in how they spread and in which treatment approaches are considered.
Is the word used consistently?
Not entirely, which is worth knowing when reading around the subject. Some sources treat cholangiosarcoma loosely as another word for cancer of the bile ducts in general, a usage that conflicts with what the suffix means everywhere else in tumor naming. Bile duct sarcomas that are well described tend to be named for their specific tissue instead, such as embryonal rhabdomyosarcoma of the biliary tract, which is the commonest biliary tumor in children, or leiomyosarcoma and angiosarcoma in adults. Where the broader word appears without such a label, the tissue of origin is worth confirming rather than assuming.
