Definition of tracer
A tracer is a substance introduced into the body in a tiny amount so that its path can be followed from outside. Most tracers carry a radioactive atom giving off signals a scanner detects, but dyes, fluorescent compounds and heavy stable atoms are used the same way. The amount is kept low enough that the substance itself changes nothing it passes through.
What makes something a tracer rather than a drug?
The quantity. A drug is given in an amount chosen to produce an effect. A tracer is given in an amount chosen to be detectable and nothing more, often a minute fraction of any dose that would act on the body. That distinction matters because the measurement then does not disturb what it measures: the sugar lookalike used in scanning is taken up by cells exactly as ordinary sugar would be, without altering how they work. It is also why allergic and toxic reactions to tracers are uncommon.

Radiopharmaceutical is the word for the radioactive kind, and each one pairs a radioisotope that supplies the signal with a carrier molecule that decides where it travels. A carrier behaving like calcium gathers in remodeling bone, which is what a bone-seeking radioisotope does. One behaving like sugar gathers in busy tissue, and one built as an antibody sticks to a chosen target.
Why does the choice of tracer decide the answer?
A scan shows only where its tracer went, so one scanner produces completely different pictures depending on what was injected. A hot spot on a bone study means active bone turnover, which fracture, infection, arthritis and tumor all cause. Uptake on a metabolic study means cells burning a lot of energy, which inflammation produces as readily as cancer does. Reading an imaging procedure of this kind therefore depends on knowing what the tracer follows, not only on where it lit up.
