Meaning of thyroid cancer: Subtype governs almost everything about the outlook, so two people given the same diagnosis on paper can face very different diseases.
Definition of thyroid cancer
Thyroid cancer is cancer arising in the thyroid, the butterfly shaped gland at the base of the neck that makes the hormones setting the body's metabolic pace. It is not a single disease. Four main subtypes, papillary, follicular, medullary and anaplastic, are told apart by how the cells look under the microscope and behave very differently from one another.
How do the four subtypes differ?

Why does the cell of origin matter so much?
Because it decides which tools work. Papillary and follicular tumors keep the follicular cell's defining habit of drawing iodine out of the blood, which means radioactive iodine given by mouth concentrates in them and can treat disease anywhere in the body without an operation. Together the two are called differentiated thyroid cancer for that reason. Medullary cancer arises from a different cell entirely, ignores iodine, and instead releases calcitonin, which serves as a marker for tracking the disease and is why an inherited form such as familial medullary thyroid cancer can be traced through a family. Anaplastic cancer has abandoned the features of both, taking up no iodine and making no useful marker, which is a large part of why it is so difficult to treat.
What raises the risk?
Radiation to the neck during childhood is the best established cause, whether from medical treatment or environmental release, and the risk persists for decades afterward. Inherited gene changes account for a share of medullary cases and a smaller share of the others. The condition occurs several times more often in women than in men. Iodine intake in the diet shifts the balance between subtypes rather than the total. One further factor is not biological at all: far more small papillary cancers are now detected because neck imaging is done so often for other reasons, and many of those would never have grown enough to be noticed, so a rising count of diagnoses does not by itself mean the disease is becoming more common.
