Monoclonal B-cell lymphocytosis is the presence in the bloodstream of a population of B lymphocytes all descended from a single ancestor cell, at a level below the threshold that defines chronic lymphocytic leukemia. There are no enlarged lymph nodes, no enlarged spleen and no symptoms. It usually turns up on blood testing done for another reason, and it is a precursor state rather than a cancer.
What does monoclonal mean here?
It means every cell in the population came from one original cell, so all of them carry the same surface markers and the same single type of light chain in their antibody. A healthy immune response does the opposite: infection drives many different B cells to multiply at once, producing a mixture. That mixture is what a routine count cannot distinguish from a clone, which is why the diagnosis rests on flow testing that reads the markers cell by cell rather than on the white cell number alone.

How often does it progress?
Two versions exist and they behave very differently. The low-count form is found in a surprising share of healthy older adults when sensitive equipment is used to look for it, and it almost never advances. The higher-count form, the one that reaches a hematology clinic, moves on to lymphocytic leukemia needing treatment in roughly one person in a hundred each year, a rate that does not fall away with time. Working the other way round, nearly every case of that leukemia appears to pass through this state first, so the two sit on one continuum rather than being separate diseases. A modestly raised risk of serious infection accompanies the clone even while blood counts stay otherwise normal.
