Definition of modality
Modality means a particular method or form of doing something, named as one option among several. In medicine it most often labels a kind of treatment, such as surgery or radiation, or a kind of test, such as ultrasound or magnetic resonance imaging. The word marks that a distinct approach is being referred to without saying which one.
What counts as a treatment modality?
Surgery, radiation, chemotherapy, hormone therapy, immunotherapy, physical therapy and psychological therapy are all modalities, because each addresses a problem by a different route. The word earns its place mainly when several are combined, and the phrase multimodality treatment exists for exactly that. In cancer care the three main modalities do different work: one removes a mass, another destroys cells left behind at the site, and a third reaches cells that have already traveled elsewhere. Because they do not substitute for one another, the interesting question is usually the order and the combination rather than the choice.

Choosing between them is choosing which question to answer, since each is blind to something another shows plainly. Bone that is obvious on a computed tomography scan barely registers on magnetic resonance, and ligament damage is the reverse. Cost, speed, radiation dose and whether the person can lie still all enter the decision, and a report that says a finding needs a different modality is saying the first test cannot answer it, not that it failed.
Does it mean anything else?
Two narrower senses are worth knowing. In physical therapy, modality means the physical agents applied to tissue, such as heat, cold, ultrasound and electrical stimulation, as distinct from exercise, which is treated as a separate category. In sensory physiology it means a class of sensation: touch, temperature, pain and the sense of position are separate modalities, carried by separate nerve fibers and processed along separate paths, which is why one can be lost while the others remain. The thread running through every use is the same idea of a distinct kind within a larger set.
