What does the abbreviation actually mean?
In practice HDR names a technique rather than a quantity: high dose rate brachytherapy, where a radioactive source is driven along a fine tube into an applicator placed against or inside the tumor, left in position for a few minutes and then withdrawn into its shielded container. Nothing radioactive stays behind, and treatment is delivered in a small number of visits. This is set against low dose rate work, in which weak permanent seeds are implanted and give out their radiation slowly over months. The distinction is about the speed of delivery, which is a property of the source, and it has nothing to do with the total prescribed.
Why can larger doses be given than before?
The limit on radiation therapy has always been the tissue surrounding the target rather than the tumor itself. Raising the dose is therefore only possible in step with better aim. Scans taken at the moment of treatment, beams shaped to the outline of the target, and methods that account for a tumor moving as the patient breathes have all narrowed the margin of normal tissue that must be included. Techniques such as image-guided radiation therapy are what allow fewer and larger treatments, so the precision comes first and the higher dose follows from it, not the other way around.
