Definition of GnRH
GnRH stands for gonadotropin-releasing hormone, a signaling molecule made by the hypothalamus at the base of the brain. It travels a short distance through a dedicated set of blood vessels to the pituitary gland, where it triggers the release of luteinizing hormone and follicle-stimulating hormone. Those two hormones in turn drive the ovaries and testes.
Why does the pulse pattern matter?
The hypothalamus does not release GnRH in a steady stream. It fires in short bursts, and the pituitary gland reads the spacing and size of those bursts as a message. Slow, widely spaced pulses favor release of follicle-stimulating hormone, while faster pulses favor luteinizing hormone, which is part of how a single molecule controls two different outputs. Constant exposure removes the message entirely. The pituitary cells respond by pulling their GnRH receptors off the surface and going quiet, so a signal that normally stimulates ends up shutting the system down.
How is that switch used as treatment?
Medicines built to imitate GnRH exploit the shutdown rather than the stimulation. A gonadotropin-releasing hormone agonist is given continuously, by depot injection or implant, so the pituitary is bathed in signal instead of pulsed with it. Luteinizing hormone and follicle-stimulating hormone fall, and with them the sex hormones made by the ovaries and testes. That drop is the point in prostate cancer, in hormone-sensitive breast cancer, in endometriosis and fibroids, and in central precocious puberty. Agonists produce a brief surge of hormone before suppression sets in, sometimes called a flare, because the first sustained dose still stimulates before the receptors disappear. Antagonist drugs block the receptor directly and lower hormone levels without that early rise.

Rising sex hormones feed back on the hypothalamus and pituitary and damp the signal that produced them, which is what holds levels within range. That loop explains why suppressing GnRH signaling lowers sex hormones everywhere at once rather than in one organ, and why the effects of low estrogen or low testosterone follow. It also explains what a blood test can separate: measuring luteinizing hormone and follicle-stimulating hormone says something about the pituitary, while measuring estrogen or testosterone says something about the gland below it.
