Definition of fertile
Fertile describes the capacity to bring about a pregnancy: to release a viable egg and conceive, or to supply sperm able to fertilize one. It rests on several systems working in step, including hormone signaling, the production and maturation of eggs or sperm, and reproductive organs that are structurally open and intact.

A problem at any one of these points reduces the chance of conception even when everything else is normal, which is why evaluation of the reproductive system examines a couple rather than one person. The word is used both of an individual and of the combined potential of two partners.
Is fertility fixed?
No. Fertility changes over a lifetime and can change within months. Eggs are present in a finite number from birth, and both their number and their quality decline with age, so this side of the equation falls more steeply and earlier. Sperm production continues far longer, though quality drifts downward too. Illness affects it: thyroid disorders, uncontrolled diabetes, disorders of the pituitary and conditions causing irregular ovulation all interfere. So do surgery involving the pelvis or reproductive organs, infections that scar the tubes or ducts, and certain cancer treatments, which is why the question is often raised before such treatment begins. Being fertile at one point therefore says little about a later one.
How is being fertile different from carrying a pregnancy?
They are separate steps. Fertility ends at conception; what follows depends on implantation, on the structure of the uterus, on the placenta forming properly and on the hormonal and immune environment supporting it. Someone can conceive readily and still experience repeated early loss, and someone whose difficulty lies in conceiving may carry a pregnancy without incident once it is established. Within each menstrual cycle, conception is possible only during a limited span of days around ovulation, so the word also describes a phase of the cycle and not only a person.
