Definition of essential thrombocytosis

Essential thrombocytosis is a condition in which the bone marrow produces a persistently elevated number of platelets, the blood cells responsible for clotting, in the absence of an underlying reactive trigger such as infection, inflammation, iron deficiency, or a recent bleeding episode. It is classified among the myeloproliferative neoplasms, a group of disorders in which the bone marrow overproduces one or more blood cell types.

An acquired mutation in a gene such as JAK2, CALR, or MPL is found in many cases and is thought to drive the abnormal platelet production, and the condition is frequently identified incidentally through a routine blood count rather than because of symptoms.

Because a markedly elevated platelet count can increase the risk of abnormal blood clotting, and in some situations bleeding, essential thrombocytosis is generally followed over time and treated when warranted with medication that lowers platelet count or reduces clotting risk, an approach based on individual risk rather than the platelet count in isolation.
