Definition of epidemiology

Epidemiology is the study of how often disease occurs in groups of people, whom it affects, and what influences its distribution. It measures patterns across populations, places and periods of time rather than diagnosing or treating any single person, and it uses those measurements to identify causes and to judge whether control measures work.

What questions does epidemiology ask?

Every investigation starts with three plain descriptive questions: who is affected, where, and when. Answering them produces two basic measures. Incidence counts new cases arising in a group over a stated period, and prevalence counts how many people have the condition at a given moment. A long lasting illness can have low incidence and high prevalence at the same time, which is why the two are never used interchangeably.

Description then gives way to analysis. The field asks whether an exposure and an outcome occur together more often than chance would explain, and whether that link survives once other explanations are accounted for. A factor that travels with both the exposure and the outcome, such as smoking in a study of alcohol and heart disease, is called a confounder, and controlling for it is much of the practical work. An association on its own is not proof of cause, and the strength of a finding is judged on consistency, dose response, the order in which events occurred and biological plausibility.

The discipline sits beside clinical medicine rather than inside it: one describes what happens across thousands of people, the other applies that knowledge to the person in the room.
