Definition of disulfiram
Disulfiram is a medication used in the treatment of alcohol use disorder. It blocks aldehyde dehydrogenase, the enzyme that clears acetaldehyde, the first and most toxic product of alcohol breakdown. If alcohol is taken while the drug is active, acetaldehyde builds up instead of being disposed of, and the resulting reaction is deliberately unpleasant.
Why does blocking one enzyme do this?
Because the body disposes of alcohol in two steps, not one. The first converts it into acetaldehyde, a substance considerably more toxic than the alcohol itself. The second, carried out by the blocked enzyme, converts acetaldehyde into acetate, which is harmless and easily used. Normally the second step runs fast enough that acetaldehyde never accumulates. Take that step away and it does, within minutes. The same biochemistry occurs naturally in people who inherit a low activity version of the enzyme, who flush and feel unwell after even small amounts of alcohol for exactly this reason, which is the clearest evidence that the acetaldehyde itself is what produces the reaction.

What else does it interact with?
More than the label suggests. Alcohol hides in many everyday products, and mouthwash, cough preparations, some sauces and vinegars, and certain topical preparations have all been reported to set off a reaction. Beyond alcohol, disulfiram slows the metabolism of several other drugs handled by the same liver pathways, so their levels can climb; blood thinners and anticonvulsants are among those affected. It also inhibits an enzyme in the production of one of the body's own signaling chemicals, which is the likeliest explanation for its occasional effects on mood and thinking. A separate antibiotic used for certain infections produces a similar acetaldehyde reaction with alcohol by a comparable route.
