Definition of diazepam
Diazepam is a benzodiazepine, a medicine that strengthens the effect of GABA, the main calming signal in the brain and spinal cord. The result is reduced anxiety, drowsiness, relaxed muscle and a raised threshold for seizures, which is why one drug is used for anxiety, muscle spasm, seizures, alcohol withdrawal and sedation before procedures.
How does it work?
GABA is the nervous system's main inhibitory messenger, the signal telling nerve cells to fire less. It acts by opening a channel that lets chloride into the cell, which makes the cell harder to excite. Diazepam does not open that channel itself. It attaches to a separate site on the same receptor and makes the channel respond more strongly whenever GABA arrives, so a brake already present is applied harder. Because GABA receptors are spread throughout the brain and spinal cord, the effect appears in several places at once, and that is why a single drug quietens anxiety, loosens muscle, brings on sleep and suppresses seizure activity. Nothing steers it toward one of those effects and away from another.

Why is it usually given for a limited period?
Two properties shape how it is used. The first is duration. Diazepam is broken down into products that are themselves active and clear slowly, so effects persist well beyond the last dose and build up with repeated dosing, particularly in older people and in liver disease. The second is adaptation. With regular use the nervous system adjusts to the extra inhibition, so the same amount produces less effect, which is tolerance, and stopping abruptly leaves the system under-braked, producing withdrawal that can include rebound anxiety, sleeplessness, tremor and, after prolonged heavy use, seizures. Combining it with opioids or alcohol deepens sedation and slows breathing, an interaction serious enough to carry formal warnings. Drowsiness, unsteadiness, poor coordination and patchy memory for events during its action are recognized side effects rather than rare surprises, and falls are the most frequent consequence in older adults. Together these are why benzodiazepines are generally directed at a defined problem over a defined period rather than used open ended.
