Definition of COPP
COPP is a chemotherapy combination of cyclophosphamide, vincristine, procarbazine and prednisone, given in repeated cycles mainly for Hodgkin lymphoma. The O comes from Oncovin, an old brand name for vincristine. It was devised as a better tolerated substitute for an earlier four-drug regimen, replacing that regimen's harshest agent, and its components still sit inside more intensive modern schedules.

The two P drugs are easily confused because the letters look identical on a chart. Procarbazine is a chemotherapy tablet with its own food and drug interactions, while prednisone is a steroid, and only one of the pair suppresses the bone marrow.
Where did it come from?
Its predecessor was MOPP, the first combination shown to cure advanced Hodgkin lymphoma and a turning point in cancer medicine. The M stood for mechlorethamine, a nitrogen mustard descended from chemical warfare research, which blistered veins and provoked severe vomiting. Substituting cyclophosphamide for it produced much the same effect with better tolerance, and that single substitution is what COPP is. The letters persist inside larger schedules that graft these drugs onto others, and in settings where this group of drugs is the affordable and available option.
What are the lasting effects?
The concern with this family of drugs extends well past the treatment itself. Procarbazine and the alkylating agents damage the cells that make eggs and sperm, so infertility is a recognized outcome, and they carry a raised long-term risk of a second cancer of the blood. Those late effects are why regimens built on a different backbone are usually preferred when an equally effective option exists, and why combination chemotherapy for a curable disease of young people is chosen with decades of survivorship in view.
