Definition of carcinogenesis

Carcinogenesis is the process by which a normal cell becomes a cancer cell. It is not one event but an accumulation of genetic and regulatory changes within a single cell lineage, each loosening a control on growth, repair or cell death. The changes build up over years, which is why most cancers surface long after the exposures that contributed to them.

What are the stages?

The classical model splits the process into three phases. They overlap in reality, but the framework explains why removing an exposure can still matter long after the first damage was done.

Which genes are involved?

Three families of genes appear repeatedly. Oncogenes are normal growth genes stuck in the on position, so one altered copy is enough to push division. Tumor suppressor genes normally apply the brakes, and usually both copies must fail before control is lost. DNA repair genes proofread the rest of the genome, so losing them raises the mutation rate everywhere and speeds the whole sequence. An inherited fault in one of these genes means a person starts life one step into the process in every cell, which is why familial cancers tend to appear earlier and in more than one site.

Why does it take so long?

A cell must acquire several independent capabilities before it counts as cancer: it must divide without being told to, ignore signals to stop, evade the built-in suicide program, keep its chromosome ends from wearing down, attract a blood supply and slip past immune surveillance. Each of those arises from separate genetic changes, and most cells that take a wrong step are destroyed before they take another. Only a rare lineage collects the full set. That is why the interval between exposure to tobacco smoke, asbestos or ionizing radiation and a diagnosis is commonly measured in decades, and why the same exposure produces cancer in some people and never in others.
