Definition of carcinogen

A carcinogen is any agent that raises the chance a cell will become cancerous. The group takes in chemicals such as benzene and formaldehyde, physical agents such as ultraviolet light and ionizing radiation, and infectious agents such as certain human papillomaviruses. Some damage DNA directly, while others encourage the growth of cells whose DNA is already damaged.

How do carcinogens act?

Two broad mechanisms are recognized. Genotoxic agents bind or break DNA, producing mutations that survive cell division; ultraviolet light and many industrial chemicals work this way, and for these there is no exposure so small that risk becomes exactly zero. Non-genotoxic agents cause no direct mutation but push already-damaged cells to multiply, through chronic irritation, hormonal stimulation or sustained radiation injury to a tissue. Many agents need to be chemically altered by the body before they become active at all, which is one reason susceptibility varies between people.

How are they classified?

Expert panels review the published evidence and place agents into groups by the strength of that evidence, not by how dangerous an ordinary exposure is. This is the most misread part of the subject: the group describes certainty, not potency.

Why does exposure not equal cancer?

Risk depends on how much, how often, how long ago, and by what route. It also depends on the person: inherited differences in the enzymes that detoxify chemicals and in the machinery that repairs DNA alter how much damage a given exposure leaves behind. Age at exposure matters, and so do other exposures acting together, which is why asbestos and tobacco smoke multiply each other rather than simply adding up. Because most agents act over decades, prevention concentrates on reducing avoidable exposure long before any sign of cancer could appear.
