Definition of barbiturate
A barbiturate is one of a family of central nervous system depressants derived from barbituric acid. Each acts on the brain's main inhibitory signaling system, and the effect climbs steadily with the dose, from mild calm through sleep to surgical anesthesia, coma and loss of breathing. Once standard treatment for anxiety, insomnia and seizures, the class now has a narrow set of uses.
How do barbiturates work?
The brain has a built in brake, a chemical messenger called GABA that quiets nerve cells by letting negatively charged ions flow into them. Barbiturates act at the receptor that ion channel belongs to, holding the channel open longer each time the natural signal arrives. At higher concentrations they stop waiting for that signal altogether and open the channel themselves. That second property is the crux of the problem: it means the effect has no natural ceiling, so each increase in dose deepens sedation until the brainstem centers that drive breathing are silenced too.
Why did benzodiazepines replace them?

Barbiturates also drive the liver to break down other medicines faster, weakening drugs taken alongside them, and regular use produces physical dependence with a withdrawal that can include seizures. Between the narrow margin for error, the interactions and the dependence, they gave way to safer options for anxiety and insomnia once those became available.
Where are they still used?
Several roles remain, all of them in settings where breathing and circulation are watched closely. Short acting barbiturates given by vein induce general anesthesia quickly and smoothly. Phenobarbital treats seizures, including seizures in newborns, and longer acting agents are used for status epilepticus that has resisted everything else. Deliberate deep sedation with a barbiturate is one option for lowering pressure inside the skull after severe brain injury. A short acting barbiturate also appears in some combination tablets for tension headache, a use that has narrowed as the risk of dependence became clearer.
