Definition of ancillary test
An ancillary test is a supporting investigation carried out in addition to the main one. In pathology it means the extra stains, antibody tests and molecular studies run on a specimen after routine microscopy leaves the diagnosis open. In a clinical trial it means a measurement taken on participants to answer a question outside the trial's original design.
What does it mean in the laboratory?
A specimen is first cut, mounted and stained with the standard pink and blue dyes, and much of the time that settles the matter. When it does not, the pathologist reaches for tests that ask more specific questions. Antibody stains reveal which proteins a cell carries and therefore what kind of cell it started as, which is how an anonymous-looking tumor is traced back to breast, lung or gut. Special stains pick out bacteria, fungi or deposits invisible on routine sections. Molecular tests read the genes directly. Some of these do more than classify: hormone receptor and growth factor receptor testing on a breast specimen decides which treatments can work at all.
What does it mean in a study?
Something different. A clinical trial is built to answer one main question, and it collects samples and data along the way. An ancillary study borrows that material to ask a further question the trial was never designed for, such as whether a marker in stored blood predicts who responded. It usually needs its own approval and its own consent, and its results carry a health warning: the trial was sized for the original question, so a finding from a side analysis is a lead to be tested, not a conclusion.

Familiar examples exist in ordinary screening. When a Papanicolaou test returns a borderline result, testing the same sample for the virus that drives cervical change is an ancillary test in the laboratory sense, added because the first test could not resolve the question on its own.
