ABVD

Definition of ABVD
ABVD is a chemotherapy combination of doxorubicin, bleomycin, vinblastine and dacarbazine, given in repeated cycles as the usual first treatment for Hodgkin lymphoma. The letter A comes from Adriamycin, an old brand name for doxorubicin. It displaced an earlier regimen that cured similar numbers of people but left far more of them infertile and facing a second cancer.

Each attacks dividing cells by a different route, so resistance to one does not carry over to the rest, and their worst effects fall on different organs. The combination runs to a fixed schedule, and keeping the full dose on time matters more to the result here than in many other regimens.
Why did it replace the older regimen?
The regimen it displaced contained alkylating drugs that damage the ovaries and testes and carry a substantial risk of causing leukemia years afterward. Since most people with classical Hodgkin lymphoma are young and most are cured, the decades after treatment weigh as heavily as the response during it. ABVD reaches comparable cure rates while leaving fertility largely intact and adding far less leukemia risk. It stands as one of the clearest cases in cancer medicine of a regimen chosen on late effects rather than on response rate alone.
What are its own late effects?
They have moved rather than disappeared. Doxorubicin weakens heart muscle in proportion to the cumulative dose received, and the weakness may not show for years. Bleomycin scars the lungs, and that scarring is permanent, so breathing tests are followed and the drug is dropped if function declines. Radiation to the chest, where it is added, brings its own long term risk of heart disease and of breast cancer. Follow up after cure therefore continues for a long time and is aimed at organs rather than at the lymphoma.
How does it compare with more intensive regimens?
A stronger multi-drug regimen used more widely in Europe controls advanced disease better but causes more infertility, more marrow damage and more treatment related leukemia. The alternative strategy keeps ABVD as the starting point and uses an early scan to decide who needs escalation, so that only poor responders receive the harder treatment. Both approaches aim at the same target: the highest cure rate with the fewest survivors harmed by the cure.
